                                    Vulnerable Sector Screening    (A.K.A. Police Checks) 

A Vulnerable Sector Screening must be done on any and all volunteers who are a member of the team coaching staff. This  includes Head coach, Assistant Coaches, Managers and Trainers. A Vulnerable Sector Screening is good for a 4 year period.

NOTE: NO team official will be permitted, on the ice or bench, until the secretary has received their VSS

  • Obtain the VSS form and get it signed by Sylvia Tremblett (President) or Greg Baxter (Vice President)

    Once you receive a signed form, ensure that:
  • both sides of the form have been completed
  • the form has been signed by Sylvia or Greg

    DO NOT USE "GMHA". For association name USE Georgina Minor Hockey Association instead.
  • Each applicant MUST BRING A SIGNED FORM IN PERSON to one of the two locations indicated on the form.  The applicant will need to show two pieces of identification, one being photo ID such as a drivers license.
     
  • Each applicant will need to pay a $15.00 fee when they submit their form to the Police (certified cheque or money order made payable to York Regional Police, Visa, MasterCard, debit card or cash).
  • Each applicant will receive a response letter directly from the YR Police, and the original must be submitted to the GMHA office in order to complete the applicant for approval. Note that the response will likely be mailed to you, so it may take a few weeks to complete this whole process.

    YOUR RETURNED VSS YR POLICE RESPONSE letter should be placed in an SEALED envelope and submitted DIRECTLY to the Secretary of the GMHA.  Please contact the Secretary to ARRANGE FOR A TIME TO submit response letter. 
  •  PLEASE DO NOT PLACE UNDER THE GMHA DOOR
